• Asthma – If you have been advised by the surgery to submit an annual review of your asthma symptoms, please use this form.
• Coronary Heart Disease / Stroke
• COPD – If you have been advised by the surgery to submit a COPD assessment, please use this form.
• Hypertension (High Blood Pressure) – If you have been advised by the surgery to submit your blood pressure readings on a regular basis, please use this form.
• Childhood Immunisations
• Routine Health Checks (Man / Woman)
• Family Planning – If you have been advised by the surgery to submit a contraceptive pill review, please use this form.
• Travel Vaccinations – minimum of 6 weeks notice required for Nurses to check records and requirements, please fill in the Travel Assessment Form before booking an appointment.
• Flu Vaccinations
• Cervical Smears
• Minor Surgery
