New Patient Registration

Kingstanding.jpgIf you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be obtained from reception.

All newly registered patients will be invited to attend a 'New Patient Health Check' with one of our nurses within the first few weeks of registering. This gives us the opportunity to meet you and your family.

The nurse will also perform some basic health checks such as height, weight, blood pressure and a urine sample.

Change of Address

If you move to a new property, please inform the surgery immediately of your new address. The receptionists will be available to advise you on whether you are still within our practice area. A change of address form will require completion.

Changes of Other Details

Please inform the practice if you change any of your details such as telephone number or surname. If you will be changing any of your name, please bring in proof of any changes as we are unable to change names without evidence.
